I typically have the demographic information for student and I have the data and information that is needed to write an IEP goal but I would like to include more of their own interests and thoughts about their speech goal. I work with preschool to 4th graders so I want to keep it age appropriate. I am thinking a section for interests, likes, dislikes, and preferences. I would want a spot where they tell why they think they need speech. A spot for them to tell what they hope to have happen in speech (their goal) "When I am done with speech I want to be able to..." I update IEPs monthly and I don't usually have students as part of that process but I would like to include them and ask how they think we should measure their progress. I would like to incorporate some pictures for the preschoolers but would like to include them in the process also. I think I can use the Vermont.gov site to help me create the template, thanks for that resource.
